Attack from the top and bottom.
Glycerine suppositories 2 4 times a day for 3 days, with oral macrogol 3 times per day.
If no change in 3 days seek medical advice.
8 macrogol/laxido/movicol in 1 litre of water drank in 4 hours will clear most impaction.
In my experience warm retention enemas-arachis oil (not for peanut allergic!!!!) For 8 hours plus oral laxatives will break an impaction.
If still no movement manual disimpaction maybe tried. This is as it sounds. The oil enema has hopefully softened the mass and a specialist nurse or Dr attempts to remove the blocking plug with fingers in a scissor movement. This is very arduous and time consuming but when done correctly when the bulk is out the bowel empties.
If the above doesn't work, its manual evacuation in theatre which maybe repeated.
The best treatment is prevention.
Personal ps-I was born with a mega colon without nerves only pain receptors. I had over 40 fecalomas removed from my mega rectum from 2010 to 2012 and 12 years on from defuncring my colon (I,have an ileostomy) I still have a huge mass trapped in collapsing mega bowel. My fecalomas were all above 11lb, my x rays look like hands holding each other but made of solid stone. The laxatives given me as a child killed any nerves I did have. This is why I chose gastroenterology, I wanted to know why I was different. And I still do not know!
